This delightful patient had some specific goals she wanted to achieve with her rhinoplasty. She absolutely didn’t want on overly “done” nose with a turned-up tip and overly scooped out dorsum. Instead, she wanted a nose that fit comfortably on her face and wouldn’t distract from all her attractive features.
Each patient has totally unique goals when it comes to what they want to achieve. I think it’s so important to sit and spend the time to understand each patient and appreciate where they want to go. I find this can’t be done with a quickie conversation, it requires getting to know the person and how they envision a rhinoplasty procedure fitting with their personality and goals.

Left Oblique:
This patient had a number of complaints:
1. Too much height to her dorsum. She wanted a reduction of this dorsal bump, but not a scooped look.
2. Her tip was turned down and the columella (this area between the nostrils) hung down a bit.
3. In addition to being turned down, her tip had a bulbous and somewhat coarse appearance. You can appreciate the changes in her tip from several different angles, including the frontal, oblique, and nose-up views.
4. Take a close look at the nose-up view and you can see how the nose had a more desirable pyramidal shape. While this view is not seen very often in day to day interactions, it reflects the more elegant shape of the tip when viewed from other angles.
5. She was particularly bothered by the droop of her tip when she smiled. Take a look at the profile smiling pictures and you can see that was nicely resolved.
6. Her breathing was not that great before this surgery, and this was greatly improved as well.

Overall, she was thrilled with her refined and natural-appearing result – and so was I. My goal is to give my patients the nose they feel they should have been born with and one that complements the beauty of their face. I think I got that done for this patient – what are your thoughts?
